Ataque cerebral agudo isquémico

Resumen

La disminución del flujo sanguíneo cerebral por debajo del umbral de autoregulación conlleva a cambios de isquemia y necrosis cerebral, que se traducen en síntomas de disfunción neurológica focal llamado ataque cerebrovascular agudo (ACA) o ictus. Dos grandes grupos según su etiología se incluyen en esta categoría el hemorrágico que constituye un 20% y el isquémico en 80% de los casos. Gran interés ha ganado el ACA isquémico por su alta carga social, siendo la tercera causa de muerte no violenta a nivel mundial y la primera de discapacidad. Muchos factores de riesgo favorecen la presentación de estos eventos y algunos de ellos son susceptibles de modificación y por tanto se  constituyen en objetivos de prevención primaria tales como el control de la diabetes, hipertensión y la práctica de hábitos saludables de vida. Los avances en el conocimiento de la fisiopatología, han llevado a cambios sustanciales en la nomenclatura y manejo del ACA isquémico. Dentro de estos cambios se sustituyó el término accidente cerebrovascular por ataque cerebrovascular agudo, haciendo énfasis en la importancia de un manejo oportuno con metas de tiempo similar al sindrome coronario agudo. Se redefinió el tiempo de ataque isquémico transitorio a una hora. Una vez instaurado el evento cerebrovascular la semiología de los síntomas con frecuencia nos permitirá realizar un diagnóstico topográfico de la lesión, que aunado al TAC cerebral nos permitirá excluir un evento hemorrágico e iniciar  tratamiento. En el manejo de estos pacientes se hace esencial la coordinación de los diferentes grupos de trabajo, desde el reconocimiento temprano de los síntomas por pacientes y familiares, la activación y respuesta rápida del sistema de emergencia y el engranaje con las instituciones prestadoras de salud. Son pilares del manejo: el abc de de la reanimación, evitar la hiperpirexia, las convulsiones, la hipoglicemia, la hiperglicemia, realizar la trombolisis en las tres primeras horas de iniciado los síntomas, usar antiagregantes, profilaxis antitrombótica, estatinas. Algunos casos específicos requerirán endarcterectomía carotídea, manejo de arritmias como la fibrilación auricular y el estudio de causas secundarias a la enfermedad cerebrovascular. De igual importancia es el inicio temprano de las medidas tendientes a lograr la rehabilitación e integración del paciente a la sociedad dentro de las cuales está el manejo interdisciplinario con nutrición, fonoaudiología, terapia física, psicología y otras especialidades.
