Cirugía toracoscópica video-asistida: por dos puertos. experiencia en Cartagena. Colombia

Resumen

Introducción: la cirugía torácica ha evolucionado, cambiando desde los grandes procedimientos invasivos a técnicas mínimamente invasivas. La toracoscopia tiene varias indicaciones para el manejo de patologías benignas y malignas.
Objetivo: analizar las características clínicas en términos de morbilidad y mortalidad, de los primeros pacientes intervenidos con cirugía toracoscópica video asistida por dos puertos (VATS).
Metodología: estudio descriptivo, observacional, tipo serie de casos de 50 pacientes intervenidos con la técnica de cirugía toracoscópica video asistida (VATS) por dos puertos entre 2009 y 2011. Se verificó normalidad con test de Shapiro-Wilk, análisis univariado con test exacto de Fisher, test de Anova y de Kruskal-Wallis para duración del procedimiento y
un modelo de regresión lineal múltiple para la estancia hospitalaria total y en UCI.
Resultados: la media de edad fue de 5 años, el 58% de género masculino, la complicación más frecuente fue sangrado postquirúrgico (6%). La patología más frecuente fue derrame pleural (28%), seguida por cáncer broncogénico (18%). Los procedimientos realizados fueron lobectomía parcial (32%), pleurectomía (28%) y lobectomía total (14%). Se requirió conversión a cirugía abierta en el (14%). La estancia hospitalaria total tuvo una media de 5.2+/-2.9 días. La estancia en UCI: 1.0+/- 2.0 días. La mortalidad fue del 2%. 
Conclusión: la cirugía toracoscópica vídeo asistida por dos puertos es un abordaje seguro, con un porcentaje bajo de morbilidad y mortalidad, aplicable para múltiples patologías torácicas y con resultados similares al abordaje toracoscópico convencional. Rev.cienc.biomed. 2012;3(1):24-30
