Factores asociados a presencia de dolor agudo postoperatorio no controlado

Resumen

Introducción: el dolor agudo postoperatorio (DAPO) no controlado tiene impacto sobre recuperación, morbilidad e incluso la perpetuación del dolor con impacto en la calidad de vida. A nivel mundial se describe presencia de dolor no controlado hasta en el 80% de los pacientes durante el postoperatorio. Frecuentemente se describen diferentes factores del paciente, del procedimiento quirúrgico o del manejo anestésico
y analgésico que pueden relacionarse con el dolor.
Objetivo: determinar la prevalencia de DAPO en una unidad de cuidados postanestésicos.
Metodología: se hizo estudio descriptivo transversal, en la unidad de  cuidados postanestésicos del Hospital Universitario del Caribe, Cartagena, Colombia. Se realizó por medio de encuesta y escala visual análoga en pacientes adultos, programados para cirugía entre agosto del 2013 y abril del 2014, aplicándolos al ingreso a recuperación
a las cuatro y 24 horas postoperatorias. Fue realizado en 265 pacientes. El  análisis estadístico se adelantó usando SPSS v15.0, comparando variables sociodemográficas, antecedentes clínicos, técnica anestésica, especialización quirúrgica y manejo farmacológico para analgesia intraoperatoria y postoperatoria, según tiempo de
evaluación y presencia de DAPO no controlado.
Resultados: edad promedio 44.9 años. La evaluación del dolor postoperatorio al ingreso a recuperación, cuatro y 24 horas postoperatorias, permitió observar DAPO no controlado, en el 14, 16 y 23% de los pacientes, respectivamente. Los factores con asociación  estadísticamente significativa fueron: tener edad menor a 60 años, nivel educativo superior, pertenecer a régimen de seguridad contributivo, haber recibido anestesia conductiva y el uso de AINES en el intra y postoperatorio, especialmente acetaminofén.
Conclusiones: la cuarta parte de los pacientes valorados tenía DAPO no controlado, a las 24 horas de la intervención operatoria. Rev.cienc.biomed. 2014(2):253-262.
