Resumen

Introducción: la hemiatrofia facial progresiva o síndrome de Parry Romberg es una enfermedad rara, de curso clínico lento, que genera atrofia de los tejidos blandos.
Compromete frecuentemente un lado de la cara, siendo el izquierdo al parecer el más afectado. La entidad se establece usualmente a edades tempranas y con el tiempo puede comprometer una o varias ramas del nervio trigémino. Se presenta un caso con diagnóstico notoriamente tardío, pese a repetidas consultas por asimetría facial y otras
manifestaciones.
Caso clínico: paciente femenino de 73 años de edad, natural y residente en área rural colombiana, con numerosas consultas por presentar asimetría facial, sin que le ofreciesen diagnóstico definido. Antecedente de trauma cigomático y del hombro derecho dos décadas antes. Dos cirugías oftalmológicas: una por cataratas y otra por glaucoma. Se observó depresión notoria en mejilla derecha con signos marcados de
atrofia, acompañado de cefaleas migrañosas periódicas y síntomas locales. Se realizó biopsia de piel donde se observó atrofia de los anexos cutáneos. No se demostraron otras lesiones neurológicas.
Conclusión: el síndrome de Parry Romberg es una entidad crónica que puede tener implicaciones oftalmológicas o neurológicas. Involucra síntomas migrañosos, parestesias y asimetría facial, que ameritan temprano diagnóstico y tratamiento adecuado, tanto de la fase progresiva como estacionaria.
